A brief welcome, thank you, and explanation

- It is for a doctoral dissertation, so I need several extra items filled out.
- This should take about 30 minutes maximum.
- Elements that are required academically for my program:
- I must get informed consent.
- I must present to you the expectations and commitments for the class.
- I must take a pre-test the performance of this “ministry project”. You will be doing this anonymously, separate from the infromed consent form.
Informed Consent
Dear Prospective Participant,
You are invited to consider participating in a twelve-week course designed to deepen your understanding and practice of biblical counseling. I will teach this course as part of my doctoral research project for Midwestern Baptist Theological Seminary and the advancement of the church’s mission in the world.
If you choose to participate, you will need to complete a questionnaire during the registration process. This process may take up to thirty minutes. Submitting the completed questionnaire is implied consent. The questionnaire is designed to assess your understanding and ability in biblical counseling before starting the course (you will complete the same questionnaire upon completing the course). A small group of participants will also be asked to fill out a survey to evaluate the instructor's effectiveness at weeks one, six, and twelve.
No benefits accrue to you for answering the questionnaire. Still, your responses will be used to establish an average level of knowledge among church members and to measure the course's effectiveness in preparing them to understand and practice biblical counseling. Any discomfort or inconvenience to you derives only from the time it takes to complete the survey.
Great care is being taken to ensure anonymity on the questionnaires and surveys you complete. This allows you to answer honestly and completely confidentially. Any information that could identify you as a participant will remain confidential and will not be shared. Your decision whether to participate will not prejudice your future relationships. If you decide to participate, you can stop at any time without prejudice.

- Complete the pretest and posttest fully and honestly.
- Attend twelve ninety-minute classes.
- Actively engage in class discussions.
- Participation in this course is completely voluntary. Inform the instructor if you must withdraw from the class.
- Participants may be invited to provide feedback on the content, clarity, and impact of the course. This would include filling out a survey.
- Participants are invited to provide feedback to the instructor at any time.
